Building on this progress, scientists continue to work on new medications and discover new ways to improve the effectiveness, accessibility, quality, and cost- ­effectiveness of treatment for people who have alcohol use disorders. Standing on the Shoulders of Giant Studies on  Treatment of Alcohol Use Disorders A massive study published in 2006 followed more than 1,300 study participants at 11 academic locations over a three-year period to determine what combination of treatment, medication, and counseling was the most effective for treating alcohol abuse disorders.?? The Combining Medications and Behavioral Interventions for Alcohol Dependence (COMBINE) study produced some surprising results when it revealed that one of the newer medications used for the treatment of alcoholism failed to improve treatment outcomes on its own.?? As shown in COMBINE, no single medication or treatment strategy is effective in every case or in every person. Anti-Alcohol Drugs Revia, Vivitrol and Campral The study did find that when combined with a structured outpatient medical management intervention consisting of nine brief sessions conducted by a healthcare professional that the alcohol-deterrent medications Revia and Vivitrol (naltrexone)  and up to 20 sessions of alcohol counseling were equally effective treatments for alcoholism.?? These results demonstrate that either naltrexone or specialized alcohol counselingâ€" with structured medical managementâ€" is an effective option for treating alcohol dependence, said Mark L. Willenbring, M.D., director, division of treatment and recovery research, National Institute of Alcohol Abuse and Alcoholism. Although medical management is somewhat more intensive than the alcohol dependence interventions offered in most of todays health care settings, it is not unlike other patient care models such as initiating insulin therapy in patients with diabetes mellitus. The study found that combining another alcohol-deterrent drug Campral (acamprosate)  with the medical management program did not improve outcomes. Campral did not perform better than the placebo or dummy pill. This finding stumped researchers since previous studies performed in Europe using Campral had yielded positive treatment outcomes. COMBINE Study Highlights After 16 weeks, the COMBINE study showed overall positive outcomes for study participants.?? All groups studied substantially reduced drinking during treatment. Overall percent days abstinent tripled, from 25 to 73 percent, and alcohol consumption per week decreased from 66 to 13 drinks, a decrease of 80 percent.Patients who received medical management plus either Revia or Vivitrol (naltrexone) or specialized counseling showed similarly improved outcomes of alcohol abstinence (80 percent) compared with patients who received medical management and placebo pills (75 percent).Patients who received Revia or Vivitrol reported less craving for alcohol.Adding either Revia or Vivitrol or specialized alcohol counseling to medical management almost doubled the chance to do well. Combined Behavioral Intervention for Alcoholism Treatment Combined behavioral intervention (CBI)â€" which includes counseling integrated cognitive-behavioral therapy, motivational enhancement, and techniques to enhance mutual help group participationâ€"has shown beneficial outcomes in studies.??   The Use of Medication in Treating Alcohol Use Disorders According to research, medications seem to be a positive part of the winning combination of alcohol use disorders. And, it is underused as a method for treating alcoholism.?? The most robust finding in the study is that those receiving any medication did much better than those who received no pills at all, says Professor Barbara Mason, Scripps Research Institute, and an author of the study. This should be a wake-up call. With less than one percent of those seeking help for alcohol dependence receiving a prescription, medication is underutilized. Medication for alcoholism can offer patients an advantage for their recovery, especially in a real-world setting. The Sinclair Method In 2001, David Sinclair, Ph.D., a researcher in Finland claimed an 80 percent cure rate for alcohol dependence when anti-alcohol drugs Revia or Vivitrol are prescribed according to his Sinclair Method. Dr. Sinclairs research has been published in the peer-reviewed journals  Alcohol and Alcoholism  and the  Journal of Clinical Psychopharmacology.?? The Sinclair Method is the standard treatment protocol for alcohol dependence in Finland, the method is also used in the U.K., but the method has yet to catch on in the United States.   With the Sinclair Method, people only take Revia or Vivitrol before  drinking  and never otherwise.?? Revia and Vivitrol are not like other anti-alcohol drugs that cause  intense sickness and hangover sensations when taken with alcohol. The change in behavior only appears over time. With the Sinclair Method, Revia or Vivitrol is taken one hour before drinking alcohol. At the end of four to six months of treatment with the Sinclair Method, 80 percent of people who had been overusing alcohol is either drinking moderately or abstaining entirely.?? The way it works is when people normally drink alcohol, endorphins are released into the brain, and this reinforces the behavior of drinking alcohol. Revia and Vivitrol block the feel-good endorphins. Much like when Pavlovs dogs were presented with food when a bell was rung, these dogs became conditioned to salivate at the sound of the bell alone. However, when these dogs continued to be presented with the ringing bell and no food, the salivating stopped. It is believed that the main reason the Sinclair Method has not caught on in the U.S. is two-fold. In the U.S., 12-step programs seem to dominate treatment plans prescribed by doctors, that, and doctors do not like that the Sinclair Method encourages people with alcohol dependency problems to continue drinking.??

Oklahoma Regents Release Statement After Waiver Loss

posted on September 3, 2014 Oklahoma State Regents for Higher Education Chancellor Glen D. Johnson released the following statement today regarding the Regent’s role in certifying our PASS standards College- and Career-Ready,which could enable Oklahoma to reapply for the NCLB waiver: At the State Regents’ meeting today, I provided an update on our goals with respect to higher education’s timeline to complete a comprehensive and efficient review of the K-12 Priority Academic Student Skills (PASS) standards, as required by HB 3399. Under the provisions of the legislation, the State Regents are tasked with the responsibility to certify whether or not the PASS standards for English and Language Arts (ELA) and math meet college and career readiness standards. To discharge their responsibilities under HB 3399, the State Regents have assembled groups of campus subject matter experts with experience in developing and reviewing ELA and math standards to assess the alignment of PASS standards with the ACT College and Career Readiness Standards. The team reviewing the math PASS standards initially met on  July 21, 2014. The team reviewing the ELA PASS standards initially met on July 29, 2014. These teams are conducting a thorough analysis, which will include the identification of any deficiencies and recommended improvements, if needed. As a part of this process, the State Regents are currently engaging consultants from the Southern Regional Education Board (SREB) to participate in the review, to provide additional expertise, and to validate the process and recommendations. As the standards review process continues to unfold, the findings and recommendations of the committees and consultants may identify additional work that could require more time. HB 3399 does not direct the State Regents to meet a particular timeline. Each step of this review will be executed as efficiently as possible without compromising the integrity of the process. Our goal is to complete the review process and present a final report to the State Regents for consideration and approval at their  October 16  meeting. The story of colonization in America begins with Jamestown in the spring of 1607. The first colony to be established, it was founded by the Virginia Company—a private corporation—and later converted into the royal colony of Virginia. The British were motivated by the allure of having the first permanent settlement and hoped to gain valuable commodities in their venture such as furs, lumber, pitch, and tar. However, these first colonists faced great challenges. They chose an area on a peninsula, hoping the surrounding water would provide defense against possibly Spanish attacks. Indeed, though, they also unknowingly chose a marshy site that bred diseases such as malaria, typhoid, yellow fever, and dysentery that fatally infected colonists by the droves. Antonio Gramsci’s hegemony defined as the dominance of one group over another has been occurring since the beginning of time. It is where a dominant force exerts its influence in society and exert their own power over other groups. Hegemony is seen with the cultural dominance of male over female. Because of this dominance men have assumed the right to determine what is masculine and what is not. With hegemony, here will be always an opposing force that plans a strategy in order to combat and defeat their adversary. Homosexuality is often depicted as inferiority. Heterosexual men see homosexual men as adversary to be defeated. Men has defined their masculinity by very specific set of behavior, habits and activities acting strong, being involved in sports and becoming engineer or doctors. Any men who do not behave according to these characteristics is viewed as â€Å"effeminate†. At an unfortunate moment with feeling between the two opposing sides ata peak, officials in Hillsborough seized a Regulators horse, saddle, andbridle and sold them for taxes. Outraged, a band of Regulators rode intoHillsborough, rescued the horse, and before leaving town, fired several shotsinto Edmund Fannings house. Fanning, who was in court in Halifax,immediately ordered the arrest of three Regulators who played a big role inthe Hillsborough horse incident, William Butler, Peter Craven, and NinianBell Hamilton. Citizens of Orange County were very sympathetic with theRegulators. Hermon Husband was chosen as one of two delegates to meet withofficials to discuss the incident. Before the meeting could be held, Fanninggathered a handful of armed men and assisted the sheriff in arresting WilliamButler and Hermon Husband. The two men were charged with inciting the peopleto rebellion and were confined in the Hillsborough jail. The governors secretaryinformed the protestors that Governor Tryon would receive their petition toinvestigate conditions in Orange County and would see that they received fairtreatment at the hands of county officials. Due to this incident, support forthe Regulation movement spread (6). The Regulators pursued their purpose with tremendous force. They oftenbroke into courts of justice, drove judges from the bench and set up mock trials. They dragged unoffending attorneys through the streets almost until deathand publicly assaulted peaceful citizens who refused to express publicsympathy for the Regulation. In September, 1770, Judge Richard Henderson waspresiding over the superior court in Hillsborough when a mob of one hundredfifty Regulators, led by Husband, armed with sticks and switches, broke intothe courthouse, attempted to strike the judge, and forced him to leave thebench. They next attacked and severely whippped John Williams, a practicingattorney. William Hooper, who later would be a signer of the Declaration ofIndependence and an assistant attorney general was dragged through thestreets to be humiliated and violently abused. Edmund Fanning was pulledfrom the courthouse by his heels and dragged from the courthouse before beingbrutally whipped. The mob then broke into Fannings house, burned hispapers, destroyed his furniture, and demolished and burned the building. Many others were whipped as the Regulators rioted through the streets ofHillsborough. Windows of private homes were broken and the inhabitants of the town wereterrorized. Court was adjourned when Judge Henderson was unable to keep order(7). The assembly of Governor Tryon set about at once to draw up a series ofreform measures. Acts were passed dealing with the appointment of sheriffs andtheir duties, fixing attorneys fees, regulating officers fees, providingfor more speedy collection of small debts, and the creation of the countiesof Wake, Guilford, Chatham, and Surry in the areas of the region where theRegulators were the most numerous. These laws were designed to meet thedemands of the Regulators, but while the assembly was vigorously passingthese laws word arrived that the Regulators had assembled in CumberlandCounty and were preparing to march to New Bern, the current capital of NorthCarolina and residence of Royal Governor William Tryon. A complete changecame over the assembly and thoughts turned toward punishing measures (8). The assembly adopted the Johnston Act introduced by Samuel Johnston, who would later be a member of the Continental Congress and a senator from NorthCarolina in the First Congress of the Unit ed States. This act was to beenforced for one year only. It stated that the attorney general couldprosecute charges of riot in any superior court in the province. All whoavoided the summons for court for sixty days were declared and liable to bekilled for treason. In addition to these drastic steps, the governor wasallowed to call the militia out to enforce the law. The Regulators, asanticipated by the governingauthorities in North Carolina, reacted withdefiance. To promote and strengthen their organization they sent messengersto nearly every county to encourage supporters and organize those who wouldjoin them. The people of Rowan County were extremely cooperative due totheir hatred of the Johnston Act (9). Governor Tryon, in March 1771, ordered a term of superior court to be heldin Hillsborough, but judges filed a protest with the council. Under the riotousconditions existing in that part of the province, they felt that they couldnot hold court with any hope of prosecution. They also feared for theirpersonal safety because of what previously occurred in Hillsborough in thecase of Judge Richard Henderson. After this appeal had been made, thecouncil decided that it was time to take a stand against the lawlessness ofthe citizens (10). Protest from the Regulators came strongly, but Tryon paid no attention. On March 19, 1771 he called for volunteers for the militia and when enlistmentsbegan slowly he offered a payment of forty shillings. The offer helpedtremendously, and on April 23 the troops got under way. Guns, ammunition,and other equipment for these troops had been sent at Tryons request fromFort Johnston on the Cape Fear River. General Hugh Waddell had already beenordered to march to Salisbury to halt the advances of the Rowan Regulators,to retrieve the western militia, and march to Hillsborough from the west. At the Johnston County Courthouse troops from Craven, Cateret, Orange, Beau-fort, New Hanover, Onslow, Dobbs, and Johnston were joined by the Wakemilitia. They made their way to Smiths Ferry beside the Neuse River whereTryon reviewed the troops on May 3, 1771. There were 1,068 men; 151 wereofficers. Pleased with his recruitment, he broke camp and advanced towardHillsborough. EssayGovernor Tryon and the militia reached Hillsborough on May 9. GeneralWaddell left Salisbury that same day, but while crossing the Yadkin River he was metand stopped by a large group of Regulators. Waddell retreated back toSalisbury. Intending to help General Waddell, Tryon left Hillsborough on May 11 leadingthe militia through the heart of Regulator country. On the fourteenth day theyreached the banks of Alamance Creek where they rested for a day. On May 16,1771, Tryon ordered his army into battle formation. The companies fromCateret, Orange, Beaufort, New Hanover, and Dobbs counties, plus theartillery, were in the lead, followed by companies from Onslow and Johnston. With these troops Tryon set out to destroy a large body of Regulatorsreported assembled five miles ahead. The Regulators, estimated at about 2,000, were waiting for Tryons confrontation. They lacked adequate leadership, a clear purpose, efficient organization, andeven sufficient arms and ammunition for battle. The Regulators must havefelt that simply by making a display of force they could frighten thegovernor into granting their demands. Among their number were many noisy andrestless individuals and many who seemed not to realize the seriousness ofthe situation lying ahead. Earlier that week, some of the Regulatorscaptured Colonel John Ashe and Captain John Walker of Tryons militia whilethey were scouting, severly beat them, and made them prisoners. So careless were the Regulators and so unaware of the situation most of themwere wrestling and playing around when an older soldier who happened to beamong them warned them to expect an attack at any minute. Shortly after, thefiring began. Before the shooting began, the Regulators were given a choiceto retreat and dissolve their group or be fired upon. In the one hour theyhad to decide few were considering their lives. The Regulators gave noresponse and thus the Battle of Alamance began. Tryons well-equipped troops soon put the Regulators to flight. The Regulators had no officer higher than captain and each individual company foughtindependently. Tryons artillery fire was very effective in the beginning, but manyRegulators later found refuge behind trees and rocks. The Regulators weredeserted by many of their own comrades and took early leave of thebattlefield. The Battle of Alamance lasted two hours. Tryons forces lost nine to deathand sixty-one wounded, while the Regulators lost the same number killed and had alarge, but undetermined number of people wounded. Tryon took about fifteenprisoners and executed one on the spot with the idea of striking terror intothe hearts of the Regulators. This action, I believe, was uncalled forbecause of the decisive military defeat. Despite his evil display ofcharacter during the battle, Tryon had his own surgeons treat the woundedRegulators (the entire battle has been summarized from source #11). The Regulators attempt to secure reform in local government by forceapparently failed completely. The Regulators were compelled to retreat from society andlive life in the wilderness. Many migrated, some going to Tennessee and downinto the Mississippi River Valley. Others followed Daniel Boones trail intoKentucky. In fact, by 1772, just one year later, about 1,500 of the formerRegulators left North Carolina (12). The importance of the Battle of Alamance and its proper place in Americanhistory have been topics of discussion not only in North Carolina, but acrossthe country. I gathered this fact from the area from which my sources came. I noticed that the efforts of the Regulators is very similar to that of thecolonists efforts to gain independence, only on a much smaller scale. TheWar of Regulation should be regarded as one of the primary thrusts of NorthCarolinas role in the Revolutionary War. Because of the research I havedone I am encouraged to find out more about the history of North Carolina. The Battle of Alamance should be covered in every American history course simply because it illustrates the desire for independence many colonists had during this time period. Endnotes1.
